ALL PARENTS KINDLY NOTE THE FOLLOWING BEFORE FILLING THE FORM
1. Forms will be available ONLINE ONLY from Thursday, 7th December, 2023 to Tuesday, 26th December 2023.
2. Enter mobile no., then click on ‘Generate code’ option for the password. The code will be sent to the same mobile no. which is being used as
password for login.
3. After online submission of the form, a printout of the same is to be deposited in school in the drop box placed at the school gate no. 1 along with
the copies of relevant documents and payment receipt of registration fee of Rs. 25/-. Payment is to be made online.
4. Last date to submit the Registration Form along with the required documents is 27th December 2023 upto 2:00 pm.
5. If the hard copy of the duly filled up form is not submitted, the online form will not be considered.
6. Admission test will be held after 15th January 2024. The schedule will be shared with the shortlisted candidates.
7. Required Documents to be attached with the hard copy of the Registration Form.:

Birth Certificate (Authorized by Municipal Cooperation, no affidavit will be accepted)

Residence Proof (Any one): (Electricity Bill/MTNL Landline Bill/Aadhaar Card/Voter ID Card/Passport)

ID Proof of both the parents (one each): (Pan Card/Aadhaar Card/Voter ID Card/Driving License/Passport)

Proof of Father's and Mother’s Educational Qualification. (the highest/latest).

Medical Fitness Certificate.

Copy of Immunization Card.

Copy of Aadhaar Card of the child and parent.

Transfer Certificate to be provided at the time of admission.

Progress report of previous two years (or as applicable)
